The Social Security Administration (SSA) has published a revised version of Form SSA-89 (12-2020), Authorization for the Social Security Administration (SSA) to Release Social Security Number (SSN) Verification. The prior version of Form SSA-89 (02-2018) will be accepted through September 30, 2021.
DocMagic will be providing Form SSA-89 (DocMagic Form ID: SSA89.MSC) in Application and Initial Disclosure packages beginning January 14, 2021 in place of the prior defaulted version (DocMagic Form ID: SSA8913.MSC). The revised form will have form features available that can be used for custom programming, similar to those available on the current form. Please note this will also be an update to the existing DocMagic Form ID SSA89.MSC to the most current template dated 12-2020.
The SSA-89 continues to require a wet signature by the SSA so will not be available for ClickSign by default.

UPDATE:
Based on further review and customer feedback, DocMagic will be updating the Form SSA-89 (DocMagic Form ID: SSA89.MSC) to a one-page document.
DocMagic will provide the revised Form SSA-89 as of February 1, 2021.
